What to bring with you 

In order to receive some services from the Florida Department of Health in Highlands County (DOH-Highlands), you must meet financial eligibility requirements.  Please bring the items listed below with you to your appointment:

  • Identification card: A picture ID is best, but a voter registration card, birth certificate, infant hospital records, pay stub, or passport also work.
  • Social Security card
  • Any insurance card(s)
  • Eight (8) weeks most current/consecutive pay stubs
    • If recently terminated, bring your final pay stub or a statement on company letterhead stating you are no longer employed there.
    • If unemployed, bring a copy of your unemployment income statement, food stamp statement, SSI statement, retirement statement, etc.
  • If you have no income, bring a notarized letter from the person or agency that is paying your room and board, or have that person come with you to your appointment and sign the letter here.

DOH-Highlands offers a discounted or sliding fee scale to those who qualify. You will not be denied services due to inability to pay.
